Application must be completed by the individual(s) named on the vehicle registration. When registered to a corporation, it must be completed by a manager or officer of the corporation. Applications submitted by third party consultants or auditors will not be accepted.

This confirms that you are an authorized third party and will be submitting the application on behalf of a carrier. If this is correct, then click 'Confirm'.
If you are not an authorized third party, then you must remove the MVID from the 'Third Party MVID' field on the Introduction Page and ONLY enter the security code. Click 'Cancel' to go back to Introduction Page to continue the online process.
