SAFETY FITNESS CERTIFICATE (SFC) RENEWAL APPLICATION

Application must be completed by the individual(s) named on the vehicle registration. When registered to a corporation, it must be completed by a manager or officer of the corporation.
Applications submitted by third party consultants or auditors will not be accepted.

To SUBMIT the application, you will need the following:

  • MVID: The motor vehicle identification number (MVID) is located on your vehicle registration. NOTE – You MUST have a commercial vehicle for which the certificate is required registered before submitting your application.
  • Insurance: Information such as the insurance provider (not agent/broker), policy number, and amount of coverage.
  • Credit Card: The application fee is $110 (Visa, Visa Debit, MasterCard, Debit MasterCard, American Express, and pre-paid credit cards are accepted).
